The easiest, most stress free tryout around. No timed 40-yard dash. No multiple choice or trick questions. No having to perform in front of Simon. Each month an individual or group will be named to the All-Health Team. To be considered for selection:

Identify a health issue that is important to you.

Using that health issue, build a project that does as least one of the following:
-
Promotes that health issue in your community,

Educates others about that issue,
-
Advocates for that issue,
-
Researches that issue.

Tell the All-Health Team about your project by completing and submitting the All-Health Team application form.

All-Health Team Monthly Competition Application
(You will need Adobe Acrobat Reader to view the application. You
can now fill out the application form electronically, print it, and mail it
in with your project summary.)
To be considered for selection during the following month, applications must be received by the 25th of each month. Each month, the All-Health Team partnership will review all of the applications received to date. From these applications, they will select one project for recognition based on the following criteria:
- Originality of project,
- Clearly stated goals and activities,
- Realistic plan of action and results achieved,
- Education/outreach component,
- Proposed resources/expenditures appropriate.
Outline for All-Health Team Project Description
(Please limit to 1-2 pages)
Please send us written information which includes the following:
- An introductory paragraph about your project. Include some history.
- The health issue(s) your project addresses.
- The role of various participants, highlighting the role played by students and achieved results.
- The benefit(s) for the students and or/the community.
- How will you publicize and share the results of your project with others?
- How will you use the money to further your project? (Please include a brief budget.)

Monthly winners will receive:

A one-time award of $500 to improve and expand their health activities or further their education,

All-Health Team T-shirts,

A prominent spot on the All-Health Team Web site,

A 30-second television spot highlighting their project,

The opportunity to be named First Team All-Health and win the top three prizes of $2,000, $3,000 or $5,000.
